Power of Attorney & Estate Planning Documents

We provide mobile notary appointments for many commonly notarized estate planning and personal directive documents, including powers of attorney and healthcare-related forms. These appointments are often time-sensitive and benefit from careful planning, especially when family members, healthcare facilities, or multiple signers are involved.

Common Documents

Durable powers of attorney
Limited or specific powers of attorney
Healthcare proxies
Advance directives
Living wills
HIPAA authorizations
Other pre-prepared estate planning or personal directive documents

Before the Appointment

All signers must be physically present for the notarial act.
Valid, government-issued photo identification is required.
The signer must be aware, willing, and able to sign voluntarily.
Documents should be complete and ready for signing before the appointment.

Important: Parker Visual Notary Services does not draft estate planning documents, advise on which documents you need, or provide legal advice. If you have legal questions about powers of attorney, healthcare directives, or related forms, please consult a licensed Massachusetts attorney.
