Important Considerations

Signer awareness — Massachusetts law requires that a signer be fully aware and acting voluntarily. If there is any uncertainty about a signer's awareness or capacity, please consult the patient's care team and, if appropriate, an attorney before scheduling.

Valid identification — A government-issued photo ID is required. If the signer's ID is not easily accessible in a medical setting, plan ahead to have it available at the appointment.

Witness requirements — Some documents require witnesses in addition to notarization. Please confirm any witness requirements with the document's originating party in advance.

Facility visitor policies — Most hospitals have visitor and vendor entry policies. Please confirm facility access procedures and communicate any requirements to us when submitting your request.
